cigna 2nd appeal

I have been denied for the second time by Cigna.They would not except my arthritis,fibromyalgia,high blood pressure,high colesteral and other things as co-morbidities.(I have been totally disabled for 13 years) and that my bmi only showed 40 and up since last  August. My question is this I have 180 days to file for 2nd appeal should I wait till July and send my papers that show my weight at above 40 since last August?I see the Dr. every 3months any way.Also we have to have 6 month diet in the last 2 years and I was on mine almost all of 2006 till Nov.will that still count ?I'm just not sure how they go back. Please help and forgive my spelling.This whole mess has almost given me a break down. Thanks Sweeties Mom ---Sherrie
