anyone know requirements for BCBS of MI PPO?

I have BCBS of MI PPO through my husband's employer effective March 1, 2008. Does anyone know what their requirements for bariatric surgery are?

Have you checked on the new policy to make sure there is no exclusion on WLS?  that should be the first step

You really need to get a copy of his specific plan because you'll get someone here who will have it covered and you may not or vice versa. Gotta see it to believe it.
