Personal letter helped

I just wanted to post to say that the personal letter that I had sent with my paperwork to insurance was the clincher for the person who made the decision.  I used the template that can be found here on OH and adapted it to my own situation, plus I put in a little bit of a sob story too. My Nut told me this today at my final appointment.  She said that someone who was in my nutrition class got denied and she had a similar situation to mine (BMI not over 40, lightweight).  She told her to resubmit with a personal letter as she has seen them work. Thank you for all of the information and support that you all provide here at OH.  And if in doubt, send a personal letter.  It really can make the difference.

Hi monky, I'm not sure if I understand the post - did you get approval or are you still waiting on a decision? If you got approval, congrats!!!! Just curious, may I see a sample of the letter you submitted? Or where can I find the template listed here on OH? I was just denied yesterday for the consultation with a bariatric surgeon (not the actual procedure) and was hoping to appeal and send a letter as well. Any chance you can give me some pointers or at least tell me where I can find a sample to modify for my situation?? A million thanks and best of luck to you!!!
