UHC and Plastics

Does anyone know what hoops have to be jumped through to get UHC to approve plastic surgery?  I have heard 3 months of documented rashes.  My question, if there are no rashes but a pannus that passes the pubic, do I have a chance?  Are there any other symptoms that could help get approval?  Any info would be extremely helpful.
