sample letter of comorbidity

I am looking for sample letters of comorbidity for my pcp.  my pcp is extremely supportive regarding my pursuit of Lap-band surgery.  But my bmi is borderline.  my pcp has assited me in looseing over a 140 lbs with diet and exersise but I have been unable to lose any more.  I have my weight loss documented with monthly visit with my pcp.  My is insurance is Cigna (Maryland) and even with that documentation they require a letter of comorbidity.  I want to do everything I can to make sure my request is approved.  Any assistance would be greatly appreciated. Are you sure you need a "letter" of comorbidity??? I do insurance for bariatric surgeons for a living (nurse at their office) and all of my Cigna patients need co-morbid conditions, not a letter for each one. For example, hypertension--examples of BP readings and medications tried, if you need a letter, your doc should say "her hypertension remains uncontrolled at 140/90, despite the use of lisinopril and metoprolol--insert your bp's and meds of course. if you have obstructive sleep apnea--all you need is your sleep study report. You need to know what Cigna's acceptable CM's (comorbidities are) ususally they are uncontrolled hypertension, uncontrolled high cholesterol, OSA (sleep apnea), diabetes. Make sure your height has been measured recently------many of our patients are WRONG on their height. 1/2 inch can change your BMI dramatically. Have your PCP do it again and recalculate. Good luck!
