Healthy NY BC/BS

Hey everyone! I am wondering if anyone had had any success with Healthy NY BC/BS?  I am hoping to get my band this year first surg appt. is Valentines Day.  Any information about what they required who they recommended ( I am in Suffolk County) would be greatly appreciated.... Thanks Jen :)
