DakotaCare?

My husband's insurance through his job is kicking in this month, and I'm wondering if anyone here has any experience with them

 
DakotaCare is the insurance company, and it lists WLS on their package info they sent home with him, stating "one surgery per covered individual or family member in a lifetime".  Does anyone have any experience with whether or not they actually APPROVE the surgery?  Which surgeries they'll approve?

I'm hoping for the DS surgery.  I've done my homework (been researching this for about 2 years now), and the DS is the best fit for me. 

Any ideas on DakotaCare's policy?  Who should I talk to?  Should I just go looking for another insurer right now?  lol
