Any Suggestions my Medical Group requirements

Just wondering if anyone has experienced this.  I have Pacificare  of Ca (HMO) in July 07 my Dr. (PCP) put in for a referal to a surgeon for RNY.  Few days later I get a call from "Healthyroads" ,for those not familar, it is Weight Managment Counciling by phone, which Pacificare required be done for 6 months before seeing a surgeon.  So I started my 1 x per week  counseling calls in Aug 07 , 4 months into this my counsler calls to tell me Pacificare no longer reqs these "calls" I tell her I want to continue just in case ,which I'm still doing. But I called Pacificare to get the scoop, sure enough they no longer require it. I called the ins gal at my PCP she did'nt have any info at this time but she'll call back.  No call back, of course,  so I called my Medical Group, EMPIRE MEDICAL PHYS GROUP, located in Palm Desert Area, I was told that although Pacificare dropped that requirement they still have it. In fact they would'nt even acknowlwdge the 4 mos spent with Healthyroads. After many "brain picking" phone calls to the surgeons office (ins gal is very nice), whom I'm not even an "official" patient of yet ,I called my medical group again this time I'm told that they have always required 6 mos Dietician Dr supervised visits even with "Healthyroads"   Not one time has anyone told/wrote me this. I asked  where I can find this requirement in writing, she was'nt sure if it is available to me per say, but my PCP should have it.  Sooooo now I'm waiting for the PCP ins gal to fax me what they have received from EMPIRE MED PHYS GRP.  Bottom Line my PCP recomends surgery Insurance will cover it and edical Group is playing games.  So I'm gathering info to make an appeal against the Medical Group thur Pacificare.
If anyone has any info regarding EMPIRE please let me know. Also if anyone wants to share what info you included on your appeal letter that would be awesome.
