Mountain State BCBS?

Has anyone had experience with Mountain State Blue Cross Blue Sheild?  My husband's company just switched to them and according to the rep he talked to, or plan excludes it, but will approve it if medically necessary.  I wondered if anyone else had them. Thanks! Sarah
