United Healthcare anyone?
United offers many levels of coverage to employers, who then pick and choose the cost/benefit combo. Unfortunately, many employers are choosing to discriminate against obese employees to save a few bucks.
Check your Certificate of Coverage for exact details (available from the HR department) or call them, as suggested above.
Duodenal Switch hybrid due to complications.

I just got switched over to UHC and I called them today and the guy I spoke to said WLS is covered but you have to have BMI 35+. Give them a call and ask. I think there are different levels of coverage. Best wishes.
