Which Companies get to decide to have WLS Exclusions?

Hi. I've never posted on this board before. I want to WLS and am on my 6 mo. supervised diet. My DH, with whom I have insurance through, may be changing employers. Can any business decide to have a WLS exclusion to make their plan cheaper, no matter how big a corporation they are? Any help with this would be greatly appreciated.

SW / BMI / SIZE: 312 / 49.5 / 26-28W CW / BMI / SIZE: 159.1 / 25.1/ 10-12
I need to lose about 2 more pounds for a normal BMI . I still seem to be slowly losing at over 2 yrs out...so may get there yet.
