Sample Letters?

Ok, i need your help in composing a letter for my pcp to sign. my insurance needs "documentation that the pcp and the patient have a good understanding of the risks invovlved and reasonable expectations that the patient will be compllant with all postsurgical requirements"

I'm sending my PCP a fax today with the requirements from BCN and i want to include a letter for him to just sign. please help me because i'm at a loss for words as to where to start! yesterday my PCP refused to write such a letter because he felt that he couldn't in good

conscience  say weather or not i knew the risks invovled in surgery or not. Even though i've talked ot him about it and even though he's the one that suggested i have WLS.  So, to make a long story short, my file will most likely be denied today because i don't have that letter. So i need help in getting him to sign this letter and then appealing.

Any help you guys can offer would be greatly apprcated.

Hi idy, Here on Insurance help there is a form letter..http://www.obesityhelp.com/content/letterofmedicalnecessity.html...This web address should take you to it....I used this letter and took out a lot of it...Because I wanted it to be simple and to the point.. Then I called the surgeon who will do my surgery. She had me fax her the letter and his office took it from there....I wouldn't use the PCP...Although the PCP needs to be behind it... Do you have a surgeon? Have you seen her/him?..Unless your insurance requires your PCP...Do all the leg work and writting yourself.. Most insurances require a Shrink report ...So she/he will say whether or not you can understand. Kathryn
