Before I send my paperwork off

Hello everyone, I am a newbie here to the site.  I was denied lap band procedure through UHC, I am have been gathering my information from my PCP and my OBGYN, I have 7 years back of weight history, a letter from my PCP of medical necessity, a physical and letter from the lap band clinic, and I have seen on here that some people write their own personal letter.  I wanted to write my own letter as well but I'm not sure what to say and what not to say, does anyone have any comments before I begin typing this letter on bhalf of myself.  I'm faxing paperwork this afternoon to the appeals department.  Can anyone with UHC tell me what their turnaround time was after they sent in their paperwork? I'm in the same situation as you....pre-op waiting. I have written a letter to be added to my file. I said how it is hard for me to care for my daughter, and all the pain I am in. If my surgeons office said it doens't look good they will take it out.

Hi Jessica-- I've probably missed your fax submission.  But, just in case you need to resubmit, or if you want to send it in late, there's a great form letter here on OH that I used.  I got approval in 8 business days (with medical mutual, not UHC).  Here's a link to the form letter: http://www.obesityhelp.com/content/letterofmedicalnecessity. html Just be sure to customize it to your needs/history/surgery you're applying for with your insurance.
