Twice ideal weight requirement?

Has anyone had a stipulation that you have to be twice your ideal weight according to standardized weight chart to be approved for surgery?

Yes, I believe with Tricare (which I have), asks for 200% of ideal weight from the Met life tables, if you have no co-morbids.
