uhc told me  I was denied  for lack of documentation. my dr resent it. they said I was still denied and had to appeal but they are telling my dr office it is in process? and not denied.. so confused.

Are you referring to documentation of your medically supervised diet? Or the 5 year history of morbid obesity?? Your doctors office may have automatically told them to send it through to the appeal process...which may mean it is in process. Call the insurance company back and ask them exactly where your file is right now, and whether it's going to the appeal process.  Make sure you know what your insurance policy requires. If you think you've met their requirements, get a copy of your records that were submitted on your behalf, and review them to see what was sent in. If there is anything missing, or if you have any additional documentation, send it in. It couldn't hurt. Also, write a letter indicating how you have met their policy requirements. I just got approved during 1st appeal through BCBSIL, and they initially told me that there was lack of documentation. However, they accepted the documentation during the appeal process. I think that sometimes they automatically deny for the littlest things, because they want to weed people out who aren't serious. Don't give up! If you've met their requirements, you'll eventually get approved.
