Physical Activity Progress/Instruction

I have BCBSIL which requires a 6 month supervised program, including progressive physical activity and instruction. Does anyone have any advice for how that should be documented? I have completed my 6 month program, but I think that the physical therapist/athletic trainer I was meeting with on a weekly basis had poor documentation which may have led in part to me receiving a denial from BCBS. I am in the 1st appeal process now. I'm actually not at all happy with the instruction that I was given during the 6 month program, but kept with the trainer assigned to me because I thought it wouldn't look good to the insurance co to switch midstream. I need to continue building on the 6 month program I went through, so in case BCBS makes me complete another 6 months before approval I don't waste time. I want to fire that group and continue with another personal trainer at the health club I belong to. I'm worried about how to document things going forward. Does anyone have any suggestions? Did you turn in personal logs that you kept for activity to the insurance company for approval? Thanks in advance for your help.

I have the same policy and I they have amended the wording of coverage......I just realized this information today. I was denied as well but it did not specify same thing. It said basically failure to lose significant weight with active participation in non surgically program.

I have the same insurance, but no one ever said I had to send in records of me working out. I didnt have a PT b/c thats xtra $ out of my pocket so I cant even submit something from a PT.  What was the reason that BCBSIL listed for denial? What was the documentation from your PT that was submitted 1st time around?  I wonder if a letter would be sufficient....

Thanks everyone. The trainer wrote a couple of letters on my behalf, but primarily focused on whether I needed continued physical therapy for back pain that I have experienced, rather than home exercise which I think is not what my insurance wanted to see. I think that I'm going to make sure that the APN/MD and dietitian record my physical activity instruction going forward and keep my own detailed logs and turn them in. I didn't know that this trainer was an option in the beginning, and it ended up draining money from my pocket for no reason. I have problems staying consistent with my exercise program, so I think I'm going to have a training session once a month just to keep me going.
