Nutritionist wrote wrong thing and i got denied HELP!

Well I was just denied because the nutritionist wrote I was under an 18 month medically supervised diet (which I wasnt) and since she wrote it they wanted proof, which of course i dont have. I told her my dr told me low carbs when I went on metformin and she took that as medically supervised.  Its too late to fix it they already have it and said since I didnt go to the dr and get weighed every month it wasnt supervised so I was denied. If I get her to revise they will think either her or myself lied to try to get surgery I was told. Any ideas!?!?!??!?! Please help I am going crazy.

I would have the nutritionist write a letter of clarification indicating what she meant by her documentation of the 18 month diet...that your doctor advised a low carb diet during xxx date and xxx date. Also, could you have your doctor write a letter also explaining what his/her treatment plan (including diet) was for you during that period of time? Is there a note in your primary doctor's medical chart which states he/she advised you of a low carb diet? If so, then there's no way you could have made this up. Make sure you send in whatever records you have that supports this. Also, look up the medical policy on your insurance co's website and make sure you have met all of their requirements for medically supervised diet for approval. Whoever supervised your diet (if required by insurance) should write a letter clarifying the program you participated in. Good luck...and Don't give up!!
