cigna questions, help with submitting claim

I am insured under my husbands work, through cigna open access ppo.  I have met with the surgeon, gone to my pcp, had psych consult and dietician consult  and to my previous pcp.  Here was my situation.    I have been an uninsured, self paying single mother for the past 5 years.   I was seeing my pcp quite frequently, and paying cash ofcourse to her.  However,  even though I went in ALOT.....I am not sure if I went 6 consecutive months.  She is also "pro" weigh****chers.   Then I got insured under my  new husbands policy.  My old pcp did not accept cigna, so I had to find a new pcp.  I visited the surgeon before finding out my dr didnt take our insurance.  I found a new dr after my visit with the surgeon.  Now the new Dr.  is great and is advocating the surgery for me because my bmi is 41 and I have diabetes and high cholesterol.  My triglicerides were at 500!!!  But again we are finding this all out just recently.   So now  everything is done,  and I have visited and have all the info I need including two letters from pcp saying that this is necessary.  The only problem I have is that my old pcp may not be able to come up with 6 months of CONSECUTIVE dr visits for diet.  She is working on my file for me, and told me to bring her the ww weigh ins that I have.  My question is might this work for cigna if she signs off on it???   And what should I do if they turn me down???  Do I just attend the dr for the next 6 months and resubmit or is that an appeal including the new 6 month diet???  And could it happen that possibly I get someone who is sympathetic to my case and just approve me????  I mean, not that I want anymore than anyone else, but I am dying here.  I hurt everyday so bad and I just want to get on with my new life.  How come this isnt "my" choice???   Sorry for ranting, but I am like everyone else here.....nervous as heck and wanting to just get on with it..... oh yeah during my psych consult,  the Dr. said I should have no problem...he said my case was strong and that he wrote a STRONG report.  Would his opinion make a difference??? thanks for any info;o)  you guys are all great
