Has anyone had any experience with EBMS?

I have looked through the past twenty pages on here or so....don't they have a search engine?  Anyway,  I looked and it said it will cover it "IF"...type of thing.  It's taking too long to get into my first appointment with the doctor.  I really want to know what my chances are of being approved and what it will detail.  Any EBMS'rs out there?

I am also seeking approval through EBMS.  For my coverage, they require a 3 month supervised diet, documentation that you have been morbidly obese for at least 5 years (this can simply be a letter of self-statement from you or your PCP), a letter of support from your PCP, and various labs completed within the last 6 months (ie ALT, AST, BUN, Creatinine, B1, B12, Folate, Insulin), a normal EKG report, a Chest X-Ray, a letter from the surgeon stating that you have been informed of all the risks associated with the surgery, and a psyc ecal report that states you do not have an eating disorder such as anorexia or bulimia, and if you ever received treatment for drug or alcohol abuse you have to have documentation of successfull completion of a rehabilitation program. Oh, and here's a BONUS:  You can do your psyc eval through your EAP up to six sessions at no charge! If your BMI is less than 40, you have to have all of the above plus documentation of at least 2 "significant" co-morbidities. If you have all of that, you are golden..... Once the surgeon submits all the info, they have 30 days to approve or deny, but most people I know have gotten approval in about 2 weeks. Best of luck, if I can be of anymore help just let me know.
