To anyone that has been approved thru BC?BS of il.

I was wondering if anyone had a copy of the records, that shows how  their DR. wrote things out so that I can show it to mine so that she knows exactly  what they want. Any help would be apperactied (SP). I have papers from them but it doesn't state how they want the dr to word things. SO im looking for what she needs i'm her first trying to help get this done and she and I both want to try and get it done right the first time, I recently found out I have the early stages of kidney disease and she doesn't want it to get any worse. And RNY is prolly the only thing going to save my life in the long run. Thanks everyone.  ((((HUGS))))

The doc needs to write a really good med necessity letter, and add progress/chart notes to it, to make sure they have complete documentation
