"Pre-determination Letter"

Does the Pre-determination letter come from my PCP or from the surgeon? What would a Pre-determination letter consist of? Thanks

The pre-determination is usually sent in by the Surgeons office and consists of your pcp ltr, nut consult, & psych eval, and your attempts of weight loss, history, co-morbidities, etc.  It will also include the CPT code (this is the code they will bill with for the type surgery you are having).   After approval & once you get your date, they will call insurance to do a Pre-Certification for your inpt stay, and confirm nothing has changed on your plan.   I work @ a hospital & used to work @ Aetna.   Good Luck on your journey.   Kerri

Thanks a lot.  The lady I talked to on the phone from my ins. co. was pretty rude.  I'm just starting this journey and am going to a couple informational seminars from different doctors within the next couple of weeks.  I don't even have a letter from my PCP yet either, but have talked to her about it and she is very supportive.  So, I feel good so far.
