I need to know how PCP should word my chart?
I think it depends entirely on your insurance requirements for the 6 mos diet. Mine is TOTALLY ridiculous and simply stated that I had to prove I was "unsuccessful" at 6 consecutive months of dieting, either under the supervision of my PCP or through a reputable community program like Weigh****chers. I called my insurance for an explanation of what they meant by "unsuccessful", but NO ONE could give me a straight answer.
So, for me, I attend WW weekly and have them stamp my little membership booklet. Then I visit my PCP monthly where she writes in my chart MY "unsuccessful" progress and notes my exercise routines. But I haven't lost a pound because that may be interpreted as "success." So... that's it. I finish my 6 mos tomorrow (whoohoo!) so I guess we'll see if I meet the requirements.
At any rate, I'd first call your insurance and have them specify in writing what they expect from your 6 mos diet and what they need to see in your PCPs clinical notes.

This is what Cigna looks for (just to get an idea of what your insurance company may be expecting in their notes):
The weight-management program
must include monthly documentation of ALL of the following components:
Vital signs, including weight (BMI and Height if not already listed)
Current dietary program
Physical activity (i.e., exercise program)
Behavioral interventions to reinforce healthy eating and exercise habits
Consideration of pharmacotherapy with U.S. Food and Drug Administration (FDA)-
approved weight-loss drugs, if appropriate
My Physician is very detailed so the more info the better. Anything you can tell your physician that you have been doing would be great so everything is well documented. She writes exactly what foods I have been eating, how many calories, describes the intensity of my workouts, what we have discussed as far as my diet and what we should work on for next visit. I am not sure what type of wording is used. A lot of people don't go into their doctors office prepared. They tens to let the physician set the tone. They can only document the info you give them because you only see them once a month. I hope this helps.
