Decision from Employer

I received my decision from my employer Saturday.  They will pay for my surgery as long as I meet the guidelines for the 2005/2006 Plan that BCBS of IL had in place.  (They paid for it up until 01/01/07 and as long as I submitted a note from my saying I was in the process of completing the requirements-- Of course BCBS "lost" those notes but my employer will accept them and they are now on file).  The guidelines for this time are the following:

1.  Medical recoreds showing I have been Obese for the past 5 years.     DONE 2.  That I have a bmi of greater than 40.   DONE 3.  The six month diet...  I haven't been to the DR.. but, I have done Weigh****chers since Jan of 2007. 

My question is this.. Does anyone have this insurance and does WW count for the 6 month diet?  If so what do I need to send them?  Receipts from where I paid?  Journals of what I ate and the exercise I did?   Any help would be great!!
