five year bmi history?

My insurance as most require five year history so thats 2002-2007 In late 2005 early 2006 I did atkins was successful but this makes it to where during that time frame I was only at a bmi of about 31. so does this mean I am just out of luck? I think late 2006 I had a bmi of around 39 but my insurance says five years at a bmi of 40.

I have uhc choice and they do pay 100 percent once its approved.

They only require one weight per year for the past five years. Have your pcp list your highest weights for those years.
