First letter from my family dr

Ok I am new to this posting things. I have joined a group from yahoo. But this is my first post. I am having problems getting my family DR. to get my letter done. Its been 4 months I have called nonstop trying to get this done. I have been to his office at least 7 times in the past 4 months asking each time. He always says he will get it done when has a minute. Do any of you have any ideals how to write one or put one together. I want to get a copy of one make it with my own things but just need something to copy to get started. Then I will take it with me to the Drs. office and have them copy it on their letter head and have him sign it while I am there. I just have no ideal what to say or how to start it.
