First, I am sorry to hear about your work-related injury.  I hope you get better soon. As for the time length required to be approved for surgery, that will depend on the insurance carrier you have.  I know people who had their first consult and were in the operating room two weeks thereafter (scary)!  Most insurance carriers, however, take some time before they will issue approval.  Oftentimes, there are medically supervised diets, extensive testing and lab work, and various classes that many patients have to take before they will be approved for surgery.  The best thing for you to do is to check with your plan's documents (e.g. explanation of coverage or summary plan description) to find out the details. As for your company's canceling your insurance, it was my understanding that even being out due to FMLA (Family Medical Leave Act) or any other protected leave, you still would have the option of paying your premiums for your insurance to continue coverage.  I have never worked at a company that canceled my insurance because I was receiving half pay or was out due to workplace injury.  But, I really have never had a workplace injury.  Regardless, if I were you, I would check with another member of your human resources department.  I am fairly sure that you can maintain your covergae if you continue to pay your premiums. I hope you get everything worked out.
