Hi all! My name is Matilda I am in NJ and I am hoping to get banded. I contacted my insurance company and this is the list they game me for the pre-approval

Current History & Physical and procedure code

Patient height, weight and ideal weight

Diet history

Medical conditions

Consultation with the following

Medical, Surgical, Sociological, and Nutritional

Is there anything I should know before I start the process............................how long did it take you to get a surgery date and or approved for the surgery. My PCP already has me on a food plan from june I have also been working out since april of this year. But i have tried so many diets and ended up hardly losing or losing no more than 20lbs and gaining it back plus more........................any advice is more than welcomed!

Trying to workout to drop the lbs first. Wish me luck! Still not sure if WLS is right for me.

Hi there. List looks pretty standard except: What the heck is a sociological consultation doing on there? I started looking into WLS in May, had my consult in June, June & July doing consults and getting my medical records sent to me, and August tying up loose ends to send stuff to insurance.  I submitted at the end of August and got approved a couple days ago.  My approval was pretty quick though, it usually takes a couple more weeks.  I got a surgery date about a day after my approval, and my surgery is on Sept 24th.  So, for me, it took about 3-4 months, which is sort of a short amount of time compared to others. History and physical you'll get from your PCP - you may want to have them run blood tests to check for high cholesterol/triglycerides and a TSH to make sure you're not hypothyroid.  The procedure code, height, weight, ideal weight stuff will probably come from your surgeon after your first consult.  They will also probably help you compile the list of medical conditions. Diet history you prepare yourself.  I went back 5 years and listed the diets I had been over the years, and recorded my gains/losses during those times. good luck!

"List looks pretty standard except: What the heck is a sociological consultation doing on there?" Many insurance companies require a physiological evaluation in order to get approval for surgery. Is this what you (OP) meant instead of the sociological consult?
