If you call the insurance company, they should be able to tell you what the requirements or criteria are. I know I hae seen others post about this comapny, but I am not familiar with them.
Most insurance companies require you to have a BMI of 40 or more or 35 with comorbidities (high blood pressure, diabetes, etc). Most require some sort of proof of a supervised diet, some 6 months, some a year long. Some require you do this right before asking for preapproval, some say it could be as long as 5 years ago. Many want you to prove you have been obese for at least 5 years. Most also require you to see a dietician and have a psyc evaluation. Insurance companies are all different though, so I would call. I would think they will tell you what they need. Mine did.
