6 month medically supervised diet

I am still appealing my insurance and my employer, but in the meantime i am starting my 6 month medically supervised PCP diet.  I will be purchasing private insurance that covers WLS but requires medically supervied diet. I see my PCP on August 24th. What info should he be putting in is records for my insurance when the 6 months is up????? I dont need his sloppy record keeping to screw this up for me.

I would ask the insurance company exactly what  info your doc needs to record. Good luck! Andrea
