Celtic care II select ppo??????????

Hey!  Anyone out there have or know about Celtic Care II Select PPO?  I am researching this individual/private plan and need to know if I have a chance at getting this surgery paid for?  It will be tom. before I am able to speak with someone regarding the exclusions.  Desperate for info!!!  Please help!  Thanks.     Amy
