Psychological Evaluation Help

I am currently seeking lap band surgery with Dr. Bessler at Columbia Presbyterian in NYC. Does anyone have any idea exactly what has to be in the psychological evaluation or better still have a sample of a psychological evaluation (names deleted, of course) that was approved by the insurance company? My insurance in NYS Empire. Thanks in advance for any help you can give me.

I can just speak for mine and hope they are similar.  You take a test that will ask the same questions in different ways to make sure your answers are consistant.  It takes about an hour.  Things from have you every had the urge to hurt an animal to are you more of a morning person or night person.  They are mulitple choice A,B,C or None of the above sort of thing. Then an hour talking about why you want the surgery, how  you feel you got to your weight and how you think life will be afterwards.  Remember, take responsibilty at least to some degree then show the modivation to keep things off and stick to the plan.  You want to be healthy and just need the surgery to help you do that....
