BCBS HMO Blue Of Mass BMI 35 anyone get approved

I have done all the pre-work and met with the surgeon, nutritionist, psych eval.  6 months documented diet, 5 years of weight history from PCP, 10 years medical records.  Blue Cross requires a BMI of 35 but you must have to co-morbiditie conditions such as diabetes , high blood pressure, sleep apnea.  I don't have those but I do have severe GERD, urinary stress incontinence, high cholesterol. chronic hip, back and joint pain and my PCP wrote a letter of medical necessity.  What do you think my chances are of getting approved?  Anyone else have the same situation as me with BCBS of Mass?  Thanks for any of your feedback.  I really need this surgery.
