Insurance denied for 3rd time - Gary please contact me!

BCBS of Alabama has declined me for the3rd time.  They wont accept my ER visit from 2004 that shows my weight of 275#.  They said they usually ask your weight at ER visits rather than weigh you.  They weighed me because I remember.  The doctor that I saw is no longer with the hospital and he has moved out of state.  They don't know where.  For 2005, I had a medical record that stated patient is overweight but didn't list my weight.  They said a letter from the doctor would work for that just to validate that the photo is authentic and was for 2005.  I only saw that doctor once and I know he wouldn't remember me.  What can I do???
