dealing with insurance

hi everyone, i am new to this board and i have questions. i have bcbs of michigan. the lap band is a covered surgery if you meet the criteria. i meet all, but the 12 months of supervised and failed documented weight loss is with weigh****chers. i have my little weigh-in book and they even marked my $12 weekly fee, but i don't have any other prove. what i am wondering if any one else has bcbs of michigan and if they had any problem getting approved?

I have them and trust me, they have made it very difficult to get pre-approved.  The diet has to be doctor supervised, and documented thoroughly.  I've been going through this for over two years and still have not been approved. My suggestion would be to get going on the doctor supervised diet.
