I'll get right to the point. My husbands employer has a exclusion in the policy not that it will not pay but just hard restriction, like a 3 year medical supervised diet, and some type of life threating illness to go along with it.  I do not have the 3 yrs or any major sickness. I am 200lbs over weight and have borderline hypertension, plus the basis problems from having so much weight to carry on my 5'4 body frame Anyway after two denial from UHC my husband contacted  by phone what I call a big wig from the corporate who he knows because she comes to his job all the time. She knows him well as the corporate headquarters in just right down the road from where he works. Even the CEO knows him.  He is a valued employer and is told this by this woman on many occasions. Anyway to make a very long long story short. My husband contacted her last week and just told her everything about the situation. Her response was does you wife have a doctor to back up that this surgery is medically necessary. He told her yes that I have two and could get more if needed. She told him to have the letters on the job this coming Tuesday as she would be there for a special event with the directors and he is a director so he will be there in the meeting. She told him have them ready and she will take them to the corp HQ and go to bat for him with the CEO and the HR insurance director.  Now here is where I need the help. She only ask for the letters from the Doctors, Should I send anymore infor to go along with it ? Such as our personal letters, lab results on infection due to the obesity. Or should I just keep it simple and only send the dr's letters.  Should I dig and find info about the DS? My surgeon only sent a few things not my whole case. Also if I need to send more info how should I line them all up. I do not want to just hand them a bunch of paper to have to dig into. I do not want them to be overwhelmed by all the stuff. What would you do?

I think chart notes from your doctor noting your constant battle with obesity along with his letter would be good. I also think a personal letter about what living in a morbidly obese body does to you; if there are family histories of heart disease, diabetes, hypertension, high cholesterol, renal failure, or any other of the major comorbidities caused by obesity, document each one, who had it, when they were affected by it, and note that the family history makes you more genetically-susceptible to coming down with those very awful complications.  Make sure you tell them which surgery you are pursuing and why. For me, I have to SPECIFICALLY state that I want the Vertical Sleeve Gastrectomy because of my renal failure; it is medically necessary for me to NOT have a malabsorptive procedure. Just my opinion; it's worth what you pay for it.

Thank you so much for answering this post. It is very important to me to try and get the right things in this package that I am sending them. I was just not sure how to set it all up. But thanks for answering this post.
