Who has Empire Blue Cross/Blue Shield PPO?

If you do, how long did they take to approve you, and did they require 6 months of medically supervised dieting?  Thanks in advance.

I am going through the approval process right now, I was told by 3 different empire bcbs reps that the 6 month diet was not required. Did you hear differently? I have 11 months of supervised diet via LA wightloss that I can send in if they want to see specific attempts.

Thanks.  I know someone with Empire BC/BS who just had lab band and she said they didn't require it.   I am so relieved.  I was curious because not all contracts under the one insurance plan are the same.  For example, one person who has Aetna will be covered for WLS, and the other person who has Aetna is not simply because they are 2 different contracts.

Hello.  I have Empire because of my employer being based in NY.  My plan does not require the diet (although boy have I tried the last 10 years).  All plans are different.  I am sending documentation of all my attempts over the last 10 years so I would send your 11 mths with LAW to show them your at least trying.

It depends on your employer's contract with Empire BC. My paperwork went in on May 31 and I was approved on June 14, no 6 month requirement.
