It's prettyu straight forward. Some states allow appeals for any reason, unless it is a federal plan. But it may be tough to beat. Have you spoken with your HR Department yet?

Thanks for answering. I have spoken to the HR Dept and she informed me that I should appeal. I work for Michael Reese Hospital and the Insurance I have is Humana PPO. We are owned by a group of Doctors from Arizona formally known as Doctors Community but they have since changed there name to Envision(I think). The HR Dept says that we are private funded because we hire out to another company for the insurance?(I did not quite understand her). Are there any specific questions I should be asking HR(actually its our Benefits Dept we speak to). Also have you seen an appeal letter(sample) floating around for laparscopic gastric bypass. My Primary and my endocronologist whom are both from Rush/St Luke (Chicago) are trying to help me. They too are frustrated and do not understand why the Insurance co/employers exclude the surgery . They are pushing me to appeal but I need some guidance in the outline of the appeal letter.
