I need help writing an appeal letter!!!!!!!

I was just wondering if anyone could help me on what I need to put into an appeal letter to tricare.  I only get one shot and I am really worried.  I didn't meet the requirements when we submitted for approval the first time and now I do. Also my surgeon now requires a 6 month diet, so if I don't get approved this time it will be at least a 6 months or more if this appeal doesn't go through. I am 4 ft 11 and I weigh 213.6.  Tricare's requirement is 100 pounds overweight  and for my height and build they require me to be 211 with a comorbitity.  I have severe arthritis of my hip and knee.  My hip problems are from legg perthes disease , and my knee is same leg. dr says that it is from my hip not being the way it is supposed to be.  any way, any help would be greatly appreciated.  Thank you,  Layla

I was never really sure what was supposed to be in it, but one day i sat down and just poured my heart out.  I included facts and statistics.  It ended up being 3 pages.  If you can email me like your home email or something i'll email it to you,  I dont think we can do attachments here.   Feel free to take whatever you want on it.  I know ive taken some ideas from other appeals.  We're all in it together so why not.
