UHC- questions

I have just recently started my journey.  With my insurance I have the option of self notifying and gathering the information for pre-approval.  They really don’t require that much.  My weight, height, bmi, dr and hospital name address, and co-morbidities and 5 year weight history.  I have everything EXCEPT the 5 year weight history.  I normally don’t go to the dr.  I have gathered some years.  Does anyone have an opinion about what are are actually looking for is it a  'weight history' or if they are looking for a history of MO? I guess I will call again, to see if they can be of anymore help.  TIA

Hello. I have UHC and my doctors office has submitted my paperwork for approval. For the 5 year history they want a documented yearly history. I was told they want to see a 5 year history of morbid obesity.  I submitted my records that I obtained from my GYN. I go every year for a physical and they take my weight. I hope this helps.

I was afraid of that!  I have not been MO for 5 years...I mean I guess that s a good thing?  But it stinks if that’s a requirement.  I am going to my PCP tomorrow for a physical, since I haven't had on in 2 or 3 years.  I even missed 2 years (I think) of my gyn appointments.  I can get 5 years, I am sure I was seen for something by some facility in the past 5 years, unfortunately it will not show a 5 year history of morbid obesity....thanks for your response.
