Anyone have an EOB they could share?
| Hi All- I know that this may be an odd request.... I am in the process of realizing that my Insurance Company has an exclusion for WLS. Soooo, I might opt for self pay. I am going to meet with the Program Coordinator at my Center for Weight Management to discuss cost/fees. I thought it would be helpful to have the "allowed amount" from several Explanation of Benefits (EOB's) to compare and negotiate a reasonable fee. I do not want to pay more out of my pocket than an insurance company would have to pay! If you have an EOB from the time of your procedure, would you be willing to itemize the charge vs the allowed amount paid by your insurance carrier? If it is easier, feel free to fax a copy to : 603-216-5048. It would be helpful to know the name of the Insurance company, but I do not want any personal data. Just the CPT Code, Charged Amount, Amount Allowed, Amount Paid. I live in NH and am hoping to have surgery in Massachusetts. I do not know if fees vary across the US?
