UHC Out of pocket maximum

This is my Out of Pocket Maximum info:

Situation

CHOICE PLUS

NETWORK

CHOICE PLUS

NON-NETWORK

Out-of-Pocket Maximum (OOPM)

 

The maximum you pay out of your pocket, in a Calendar Year for Copayments.

 

 

 

$1,500 per Covered Person per Calendar Year, not to exceed $4,500 for all Covered Persons in a family.

 

The Out-of-Pocket Maximum does not include the Annual Deductible.

 

$4,000 per Covered Person per Calendar Year, not to exceed $12,000 for all Covered Persons in a family.

 

The Out-of-Pocket Maximum does not include the Annual Deductible.

$1,250 per Covered Person per Calendar Year, not to exceed $3,750 for all Covered Persons in a family.

 

 

 

So does this mean that if I am approved for the surgery that the most I have to pay is $2000?? ($1500 out of pocket + $500 deductible) I am having a hard time finding info on the UHC website.  Thanks for any help provided!

One other item you may want to look up in your insurance coverage book is if there is a separate charge for the surgeon.  In my case, I have to pay a certain percentage for the surgeon up to a certain amount.
