Self-pay option when WLS is excluded

My husband has United Healthcare through his employer with a specific exclusion for WLS. My husband asked if WLS would be included when his insurance renews - and was told "no." My husband will ask if a rider can be included when his insurance renews in October.  BUT - if he is told "no" again - what and where (I'm in the state of Ohio) can I get a low-rate loan to pay for WLS. I looked at a personal loan against our home equity. That was over 9% and my husband was not interested in that route. That would cost us $350/month to pay back the loan. Do I have other options?

The really big positive about the home equity loan is that the interest is normally tax deductible and is not on a conventional loan.

My husband talked with the HR insurance person at his work and she said that it wasn't possible to have a rider added to the insurance for bariatric surgery. She said that United Healthcare would not allow the rider. Does that sound correct? If so - since my husband's work has excluded bariatric surgery and they say that a rider is not possible - do I only have one option left: to get a loan to pay for surgery?  Thanks
