6 month diet? required????

I need`some help from all of you insurance pros, please!  This is my policy ..... http://www.anthem.com/medicalpolicies/noapplication/f4/s10/t 2/pw_034084.pdf I dont see that mentioned ... am I missing it??? TIA! - Summer

It does say that you have to have attempted to lose weight thru a surgeon supervised diet/exercise plan. It does not give a time period. Mine has something similar. I had to have attempted to lose weight within the last 12 months and it had to be physician documented. I did not have to have tried for s specific length of time. Luckily for me, I had several talks with my PCP about my weight in the last year and was approved. Call the insurance company and ask specifically what documentation is needed to meet the diet requirement. If it is just one document- great, but they can give you the best info on what is needed to make sure you get approved. I have found that if you are super nice to the rep, they will get you what you need. Make sure to get their name etc. and document every phone call. I hope we hear you are approved soon!

I would say no... I would still call and make sure... when it gets closer to the time to submit - check again... they can (and do) change the requirements for approval at any time and are not required to notify you!

First visit to surgeon - 288 ~ bmi 45.1
2 week pre-op 252 ~ bmi 39.5
Total lost - 153 Since surgery - 117!
Goal weight - 155 (mine) 180 (surgeons)
Current weight - 135 (2020 I lost 10lbs due to dedicating myself to working out more and being in better shape)

1/14/2025 still maintaining 135 :-)
