What happens if you s-t-r-e-t-c-h the truth a little?

I have UHC, which requires a 5 year history of BMI > 40.  I don't have that.  I do have a 5 year history of BMI > 35 ALMOST with a co-morbidity of high blood pressure.  I am only about two pounds below for a year.  The only backup I have is a Curves weigh-in sheet. My question is this, rather than be denied for being two pounds below, should I try a little White-Out and change the weight on the sheet?  What happens if the insurance company catches me at it?  What will they do?  (Other than deny me...)

You can go to jail for insurance fraud. Maybe include the curves chart for proof for that year instead of the doctor's chart. And then you still have 5 pieces of documentation for the 5 years. The insurance co. can still request more documents from the doctor, though. Also, check your height, maybe you are shorter than you think you are- this happens a lot! I truly hope you get the surgery that you need, but would never recommend that you put yourself into a situation where you can ruin your life.
