Help with Aetna 5 year history...

I am trying to get a Lap Band this summer. I have done everything needed and was under the impression that all I needed was 6 months of doctor supervised diet and that was pretty much it. Well, I got a call at noon today saying that I need to send my surgeon's office 5 years of weight history. I spent the afternoon running around town trying to collect it. I have 2007, 2006, 2004, 2003. I do not have 2005. I didn't see a doctor at all in 2005. I have a weight recorded from Dec 30, 4004...so close! Is Aetna going to tell me that I don't qualify because of this? I am very stressed. Any help would be appreciated! Thanks!

Did you get weighed anywhere for anything? Think, think, think... A gym membership, WW, School, anything that may be subbed for the missing paperwork?

HI, I have Aetna and I just want to warn you.  Make sure that all your weight is above 40 BMI for the years that you submit!!  I fell below that twice but they don't see that it went back up right after....its called diets and failed attempts......I was denied twice because of that.... Good luck, I am sure you will find it somewhere!!
