Question for all you pros :)
I called my insurance company today to find out what their requirements are up front because if I need 5 years of weight history and all I'm gonna have to do a lot of searching to find my PCP from before I moved to Indiana and all and they said that their only requirement is that I am Morbidly Obese with Co-morbidities and have Psychiatric eval and some other testing. So does this mean I may be able to avoid having to jump through hoops, do y'all think?? So far I Meet all of their requirements.
I'm still beginning my journey to an extent, have had my consult and have my psych eval on monday, I'm hoping I don't need a while lot more.
You said you called them, what did they say? If you meet all their requirements then I'm sure once you get done with your testing/jumping through hoops (psych eval, consults, etc) you'd qualify. I think you are doing all the right things to get ready for approval.
Good luck!

It sounds very similar to my insurance (United Healthcare). All I needed was 5 year history, BMI over 40 and age 25.
My doctor required the Psych eval, visit to the NUT, and an upper GI scope.
It only took me 3 1/2 months to get approved and have the surgery.
As for the 5 year history, Luckly I had two children (now 5 and 3) and thus had a good history of my weight. (I normally don't go to the doctor). So they will take any medical records with your weight. Mine were from my ObGYN
BTW - this is much better then my former employers insurance. With that it required a 12 month supervised diet, in addition to a bunch of other stuff. I did 8 months of the diet before I left that employer and moved to a different state, so I had to start all over.
