What do I need for the Appeal???

I am sending my letter to appeal since I was denied for the gastric bypass surgery. Does anyone know what information I need to take with me? What is it like at the appeal? Do I basically state all my reasons for having the surgery? Any information please??? I have title 19 in Wisonsin

You need to deal with the specific reasons why you were denied.

Call your insurance company and ask them if they have a list of requirements, this way you can make sure that you meet all those requirements when you resubmit.  Document everything and everybody you talk to!  GOOD LUCK
