Insurance, What a pain

I got my approval from Amerihealth PPO back in march but that is my secondary insurance. I have Horizon (the plan I have) for a primary and they do not cover the surgery. So, I have had to wait all this time just to hear that. I know that I am going to have a balance, does anyone have an idea of how much that usually is and does it have to be paid first? I can't seem to get any answers from the lady at Batrix. I am going to Dr. Mayamor in Langhore PA I am feeling like it just isn't going to happen.
