Hi! I was wondering if someone has a sample letter that I could use as a guide to send to my insurance co. from my pcp. My pcp wrote a letter to my insurance co. in 2003 when I was trying to get approved with another insurance. It was a very short letter that did not help me to get approved. I had heard before that many people write their own letters and have their pcps sign them. Any help is greatly appreciated! Karen

Thanks so much! Do you know if I need to write a letter myself, too? My insurance company has sent a letter stating that they couldn't approve of the surgery because I didn't have enough proof. They didn't state that I would have to appeal--they said I could send in or fax more documentation. So, I'm guessing that they haven't denied me?!? Karen
