Dietary Consults, How much did you pay?

How much did you guys pay for your dietary consults/nutrition counseling?  I called to schedule the appointment and they said it would be a $200 fee.  They said that this fee is set by the surgeon, not the insurance.  So I called my insurance company and they said that it is excessive and costs more than the surgery.  I only have to pay $100 for the surgery(I have FED BCBS) and all consults and exams leading up to it are fully covered.  The only thing I have to pay is my normal co-payment of $20 for any visits psy., dietitian, etc.  I suppose that the Dr. just has his own dietitian and the fee set.  The dietitian said that, "she is just the dietitian and she does not set the fees".  "Normally, most insurance companies don't cover it so that is why he has his set fees."  I am waiting on a call back from my Dr.  I will not pay for something that is already covered.  He is a great doctor and I do not feel that he is trying to exploit me or anything, but if that is the case I will have to find my own dietitian if he will allow it..  I mean his people is not going to tell me anything that anyone else would not.  That's what I pay insurance for so i don't have to pay excess out of pocket fees.  Let me know your experiences......
